Children aged 0-10 years are assessed by the Community Paediatric team and secondary school age children 11-16 years are assessed by CAMHS but all referrals go to a single point of access:

Autism referrals should be completed by school and require;
- Multiagency Consent and Referral Form (to be completed by school)
- Current Concerns form- (to be completed by school)
- ASD Pathway Information Agreement Consent Form (to be completed by parents)
- AQ-10 Screening questionnaire (to be completed by parents)
Preschool children can be referred in either by nursery or by the health visitor.
The services also ask school to share any professional reports e.g. Educational Psychology or SALT. If the young person is home educated then they can accept a referral from the GP and will ask the family to complete a Developmental History form before discussing at triage.
