Our Wirral Community Learning Disability Service is for adults aged 18 and over who have a learning disability and live in the Wirral.
When a new referral is received it is discussed at a team meeting, and the most appropriate member of the team is allocated to work with the person. Usually the team member will carry out an assessment to establish the person’s specific needs. This may take place in the person’s home, day service, or place of work. If necessary, the team member will also talk to anyone who supports the person; this could be family, day centre staff or care staff.
To access the community learning disability service, please speak to a GP, social worker or any other health professional working with you. They will be able to refer you if they think our service can help. Alternatively, you can make a self-referral or refer as a family member or carer.
